Consumer Information

For the Love of Therapy, PLLC provides the following information for clients and the public to receive information regarding access to health care records, contacting the state licensing authority, and about filing a consumer complaint. This information is in line with requirements from House Bill 4224 and the Texas Health and Safety Code section 181.105.

Access Health Records
 

Every client has the right to access their health care records. To do so, please follow the given steps listed here:

1.   Email For the Love of Therapy, PLLC via email at fortheloveoftherapy@gmail.com with your desire to obtain your health care records. You will receive a Documents Request Form via email. 

​

2.   Fill out the information for the client of whom the records are being requested. If you are NOT the client, please ALSO include your information as the requestor.

​

3. Any party requesting records, whether for themself or another person, will be required to provide proof of identification. Identity verification is required to protect client privacy and confidentiality in accordance with applicable state and federal laws. Further, any party requesting records for another party will be required to provide proof of legal authority to do so. This includes but is not limited to custody or guardianship records, power of attorney documents and more such items. If you are unsure about the validity of the document you want to use to prove legal authority in a health care records request, please contact For the Love of Therapy, PLLC. 4.   Requests may be subject applicable fees as permitted by law.

​

5.   Records will be provided within the timeframe required by Texas law.

Filing a Consumer Complaint

If you believe your rights have been violated or you wish to file a consumer complaint, you may contact the Office of the Texas Attorney General’s Consumer Protection Division.

This process is independent of the practice and allows consumers to raise concerns related to services provided in Texas.

If you would like to contact the Texas Behavioral Health Executive Council (BHEC), which oversees behavioral health licensing boards in Texas, you may do so using the information below:
